About The Position

The Client Outreach and Engagement staff members will act as the triage point for Law Enforcement Assisted Diversion (“LEAD”) in Albany, Cohoes, and Watervliet. As such, this position must ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals and diversions for LEAD. This position will provide person-centered case management in the community to individuals referred to LEAD. Case management services may include accompanying clients to necessary services such as homeless shelters, the Department of Social Services, court appointments, probation appointments, medical services, and meeting at a client’s home or in places in the community that have been mutually agreed upon. This position may also provide screening, outreach and engagement services on an as-needed basis. The Case Management staff members will conduct street outreach as needed to engage referred and diverted individuals who have yet to engage with their assigned case manager. The Outreach and Engagement staff must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participants.

Requirements

  • High School Diploma or equivalent (GED/ TASC) required; College highly preferred.
  • One year of experience in homeless services.
  • Flexibility to work hours outside of regularly scheduled work hours, within reason.
  • Demonstrated ability to work well with clients and cohesively as a team member.
  • Clean and Valid New York State Driver’s License; actively licensed for at least one (1) year with reliable transportation.
  • Proficiency in various computer applications: Microsoft Office Suite, Google Business Workspace Suite; and use secure Internet practices.
  • A professional record of integrity, proven reliability, and dependability.
  • Ability to work flexible hours, outside of scheduled work hours, when necessary.
  • Excellent organization, planning and time management skills.
  • Strong communication and interpersonal skills, with the ability to build strong working relationships with regulatory bodies.
  • Ability to function well in a high-paced and, at times, stressful environment.
  • Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude.
  • Demonstrates qualities of patience, flexibility and adaptability; and capable of exhibiting empathy for those in crisis.
  • Ability to exercise ethical, sound independent judgment and discretion, and handle sensitive data with strict confidentiality.

  • Previous experience working in a human services agency is a plus!

Responsibilities

  • Engage directly with youth and families to Provide 1-1 teaching, coaching, and rehabilitative counseling to build the skills and capacities of a youth struggling with mental health and developmental challenges.
  • Engage with youth in their home and in the community to ensure for a person-centered approach.
  • Provide services as identified on the youth’s Treatment/Service Plan following the specific frequency, scope, and duration.
  • Assist families to better understand and parent their child with mental health and developmental needs.
  • Attend all mandated training as assigned by their supervisor.
  • Regular and ongoing communication with other providers and family to advocate for a youth’s needs, mental health wellbeing and any non-healthcare needs (ex. housing, employment, social support).
  • Timely and accurately document the delivery of services in an electronic health record.
  • Local travel required to provide the services in the youth home, school, or other community settings.
  • Maintain a positive, strength-based and person-centered approach at all times.
  • Provide other support and rehabilitative services as the agency determines, based upon new contracts or changing regulations.
  • Additional position-related duties and responsibilities as assigned by the supervisor.
