Assists and completes needed applications; provides crisis intervention, mental health assessments and other supportive services for individuals enrolled in a housing case management program. Complete telephone intakes; develops and monitors case management plans and provide supportive services to individuals on caseload. Maintain confidentially and adhere to compliance agreements and program standards. Provides resources for basic essentials, such as food and household supplies. Perform case management sessions advocating for clients, making contact with landlords regarding landlord statements, and other housing concerns. Contact Housing Specialist to make arrangement for a lead inspection if there are children in the household, monthly request for payment of rent once client has met all requirements; complete quarterly redeterminations for program eligibility Assist in data collection and input for reports, plans program meetings, and reviews clients’ progress. Create and maintain client information in VESTA & Clarity/HMIS system, including vital statistics, and services requested and received for APR and other reports. Make weekly contact with client; monitor case management plans to insure goal achievement. Collaborate with Strategies to End Homelessness organization case managers, Social Security, Mental Health Board, landlords, and other social services organizations to ensure continuity of services. Complete referrals to various organizations or special programs and / or services to end homelessness; participate on designated committee that address diversion or homeless services.

  • Bachelor degree (preferred) or associate’s degree in human services in related field with at least two (2) years experience providing direct client services.
  • Experience preferred in providing case management services to clients providing outreach and case management services, including crisis counseling and emergency housing.
  • Able to perform assessments, advocacy, housing counseling, and community referrals.
  • Experience working with housing providers and to develop community-housing resources.
  • Able to develop and maintain accurate casework files and compliance reports.
  • Basic math and budgeting skills.
  • Ability to manage multiple tasks and work effectively with a diverse population.
  • Proficient in all Microsoft software (Outlook, Word and Excel) and other database systems and accurately input information into a database (Clarity, VESTA).
  • Good oral and written communication and strong interpersonal skills
