About The Position

Nat-Su Behavioral Health is seeking a full-time Support Staff III / Case Manager to support our Intensive Outpatient Program. This role works directly with clients to provide case management services, peer support, and skill development while ensuring continuity of care and coordination with internal teams and external partners. The position includes weekly travel to the Skull Valley Reservation, medication distribution, transportation, and active participation in the therapeutic community.

Requirements

  • CCM (Certified Case Manager) certification required
  • High school diploma or equivalent required
  • Previous case management or closely related experience required
  • Valid driver’s licenserequired
  • Peer Support Specialist Certification required within 90 days of employment if not previously obtained

Nice To Haves

  • Experience working with Medicaid programs preferred
  • Background in substance use treatment or behavioral health settings preferred
  • Proficiency with electronic health records (EHR) systems and Microsoft Office applications
  • Strong interpersonal skills and ability to work collaboratively with clients, staff, and community partners
  • Ability to respond appropriately and effectively in crisis situations
  • Proficient oral and written communication skills
  • Ability to maintain confidentiality and comply with all privacy regulations
  • Ability to work independently with minimal supervision

Responsibilities

  • Travel to the Skull Valley Reservation weekly providing case management services
  • Develop, implement, and monitor individualized case management plans in collaboration with clients, therapists, physicians, and support networks to address client’s needs and goals
  • Serve as a point of contact between clients and their social, legal, medical, and professional support systems
  • Support clients with medical, legal, educational, and employment-related appointments and relay relevant information tomedical orclinical staff
  • Assist clients in developing independent living and life skills appropriate to their phase of treatment
  • Provide crisis intervention as needed
  • Maintain presence in therapeutic community (TC) groups
  • Follow facility schedules, enforce boundaries, and promote a safe and supportive environment
  • Promotes integration of services for clients including behavioral health care and long-term services and supports to enhance the continuity of care
  • Uses motivational interviewing to educate, support,and motivate change
  • Works to create and maintain community partnerships
  • Attend all mandatory staff meetings
  • Dispense medications at designated medication times in accordance with policy
  • Record new client medications in the Notation Matrix
  • Transport clients to appointments and program activities as required
  • Update client calendars with appointments
  • Complete daily documentation in the Electronic Medical Record (EMR)
  • Assist with intake and discharge processes
  • Obtain and maintain appropriate releases of information in compliance with HIPAA and CFR 42 & 45
  • Collaborate with outside agencies to connect clients with employment, education, housing, healthcare, and community resources
  • Perform other duties as assigned
