About The Position

The Case Manager III – HUD-VASH provides advanced clinical case management and care coordination to veterans experiencing homelessness. Reporting to the VASH Team Supervisor and Project Director, this role supports veterans with complex medical, behavioral health, and psychosocial needs to achieve housing stability and independence. This is a senior level case management role with responsibilities that include clinical assessment, documentation oversight, and mentorship of staff and interns.

Requirements

  • Master’s degree in Social Work, Psychology, Counseling, or related field
  • Experience providing clinical case management to individuals with complex behavioral health and social service needs
  • Ability to obtain and maintain VA credentialing including background clearance
  • Strong clinical assessment, crisis intervention, and documentation skills
  • Excellent communication and collaboration skills
  • Proficiency in Microsoft Office and electronic case management systems
  • Valid driver’s license and ability to meet insurance requirements

Nice To Haves

  • Experience working with veterans and/or individuals experiencing homelessness
  • Substance abuse counseling certification
  • Experience supervising interns or providing clinical mentorship
  • Basic Life Support certification

Responsibilities

  • Provide comprehensive case management and care coordination for veterans with complex needs
  • Conduct psychosocial assessments, mental status evaluations, and risk assessments
  • Develop and monitor Individual Housing and Employment Plans and Mental Health Treatment Plans
  • Provide individual, family, and group counseling and supportive interventions
  • Coordinate services across healthcare, behavioral health, housing, and community systems
  • Support veterans in maintaining housing through life skills coaching, resource linkage, and crisis intervention
  • Conduct home, office, and community visits to reassess needs and progress
  • Facilitate groups such as recovery, life skills, and wellness programming
  • Maintain accurate and timely documentation in HMIS, HOMES, and internal systems
  • Ensure compliance with HIPAA and program regulations
  • Support audits, reporting, and quality assurance activities
  • Assist leadership with program reporting and data tracking
  • Provide guidance and mentorship to staff and interns
  • Assist with onboarding and training of new team members
  • Collaborate closely with clinical leadership on complex cases
  • Partner with VA providers, community agencies, and interdisciplinary teams
  • Maintain strong relationships with clients, families, and stakeholders
  • Participate in team meetings, trainings, and case conferences
  • Maintain required certifications and documentation
  • Track service delivery, expenses, and program metrics
  • Provide after hours support for urgent client needs as required
