We are seeking a Case Manager II to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. We offer work-life balance with reasonable case loads of an average of 30! This Case Manager will be based out of the Twin Cities metro area, while supporting our St. Louis clients that reside in the Twin Cities area and Central Minnesota.
