About The Position

Case Manager II We are seeking Case Manager II's to work on our Waiver Case Management Hennepin team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. We value work-life balance and have maneable caseloads that are on average 30! Hours: Full-Time, Monday- Friday, business hours 8am-4pm Locations: Hennepin County | Minnesota | Hybrid Hiring Range: $24.00 - $25.35/Hourly based on qualifications Hiring Bonus: $1000 to external candidates People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. Requirements

  • Valid driver's license, excellent driving record and current auto insurance
  • Bachelor's Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field;
  • OR
  • Bachelor's Degree from an accredited four-year college with a major in any field, AND
  • One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Responsibilities

  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
  • Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
  • Making referrals and authorizing services based on needs
  • Maintaining records and documenting all coordination activities
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards
  • Must be able to have on-site presence in the Twin Cities, Minnesota
