About The Position

The Case Manager II recruits, screens for eligibility, conducts intake services, and assists the Community Services Director in quality monitoring activities for low-income individuals and families in Burke, Caldwell, and Rutherford counties for the purpose of educating those families about the availability of resources to meet their needs. The Case Manager II assists individuals utilizing a variety of case management tools for Information and Referral, Self-Sufficiency, Circles, Head Start, Weatherization, and the Senior Services programs.

Requirements

  • Associate’s degree in Community Services, Human Services, Social Work, or a related field required; or
  • Bachelor’s degree in Community Services, Social Work, Human Services, or a related field preferred; or
  • An equivalent combination of education and relevant work experience.
  • Two to four years of professional experience working with economically disadvantaged individuals and families, or an equivalent combination of education and experience.
  • Strong organizational, communication, and documentation skills.
  • Working knowledge of Microsoft Office and internet-based applications.
  • Ability to learn agency data systems and reporting software within 90 days of employment.
  • Ability to work effectively with diverse populations.
  • Ability to work collaboratively as part of a team.
  • Ability to pass a criminal background check and substance abuse screening.
  • Valid driver’s license, reliable transportation, and proof of required insurance.

Responsibilities

  • Demonstrate strong interpersonal, communication, and problem-solving skills to assist individuals and families in identifying and mobilizing internal and external resources to improve their economic status.
  • Conduct client interviews to determine needs and eligibility for services and provide orientation to program goals and services.
  • Complete comprehensive intake and assessments, including employment, education, family composition, housing, transportation, nutrition, and other relevant factors.
  • Develop individualized and family-centered case management plans, including goal setting and measurable outcomes.
  • Provide counseling, guidance, referrals, and follow-up support to assist clients in making informed decisions and achieving established goals.
  • Maintain accurate, timely, and confidential case records, including documentation of services provided, participant progress, and supporting documentation.
  • Assist clients with completing applications and required paperwork for internal and external services.
  • Advocate for clients and maintain ongoing contact throughout program participation using various methods, including in-person meetings, phone, email, and other approved communication methods.
  • Monitor and follow up on active and inactive cases to assess progress, service utilization, and case status.
  • Plan and facilitate self-help workshops and group activities focused on topics such as employment readiness, budgeting and money management, life skills, crisis prevention, nutrition and wellness, and family stability.
  • Identify and coordinate resources available through BRCA and partner agencies, and track services, expenses, and outcomes in designated data systems.
  • Prepare required reports, outcome data, and evaluations as assigned by the supervisor.
  • Build and maintain collaborative relationships with community partners and service providers to address unmet needs and improve service delivery.
  • Assist with research and support efforts to secure funding and resources to enhance Community Services programs.
  • Perform other related duties as assigned, in accordance with agency policies, procedures, and applicable laws and regulations.
