About The Position

- The Case Manager I at LIFESTREAM BEHAVIORAL CENTER is responsible for providing comprehensive case management services to individuals with behavioral health needs. The role involves coordinating care, facilitating access to necessary resources, and ensuring the delivery of high-quality support to enhance the well-being and recovery of clients. Key Responsibilities: - Conduct thorough assessments to identify client needs, strengths, and goals. - Develop, implement, and monitor individualized service plans in collaboration with clients and their families. - Coordinate and facilitate access to community resources, services, and supports. - Advocate for clients' needs and rights within the healthcare and social service systems. - Maintain accurate and up-to-date documentation of client interactions, progress, and outcomes. - Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients. - Provide crisis intervention and support as needed. - Educate clients and their families about available resources and services. - Monitor and evaluate the effectiveness of service plans and make adjustments as necessary. - Participate in training and professional development opportunities to enhance skills and knowledge.

Requirements

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.
  • Minimum of 1 year of experience in case management, social services, or a related field.
  • Experience working with diverse populations, including individuals with mental health or substance abuse issues.
  • Proven track record in developing and implementing individualized service plans.
  • Strong understanding of case management principles and practices.
  • Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, families, and professionals.
  • Ability to assess client needs and develop appropriate service plans.
  • Proficient in maintaining detailed and accurate records and documentation.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Demonstrated ability to work independently and as part of a team.
  • Familiarity with community resources and ability to connect clients with appropriate services.
  • Competence in using case management software and Microsoft Office Suite.
