About The Position

This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, and serving as a QIDP while providing monthly program monitoring. The Case Manager is also responsible for managing and prioritizing a caseload and providing services to individuals with intellectual and/or developmental disabilities and their families. Compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines is required.

Requirements

  • All New Employees will complete the Case Manager Academy Training Program
  • For Case Manager - Level I: Bachelor’s degree in a human services field and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.
  • For Case Manager - Level II: Bachelor’s degree in a human services field and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.
  • For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
  • Must possess a valid driver's license
  • Must maintain required car insurance
