About The Position

This position is located in El Reno, Oklahoma. Under the direction of a case management supervisor, the role provides services to individuals with intellectual and/or developmental disabilities who live in their own homes or community living alternatives. Key responsibilities include obtaining in-depth social histories, developing and securing community and/or agency resources to meet consumer needs, serving as a Qualified Intellectual Disability Professional (QIDP), and conducting monthly program monitoring. The position requires adherence to Federal Home and Community Based Waiver regulations, as well as state and policy guidelines.

Requirements

  • Complete the Case Manager Academy Training Program
  • Possess a valid driver's license and maintain required car insurance
  • For Case Manager - Level I: A bachelor’s degree in a human services field and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities.
  • For Case Manager - Level II: A bachelor’s degree in a human services field and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities.
  • For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.

Responsibilities

  • Provide services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives
  • Obtain in-depth social histories
  • Develop and assure community and/or agency resources to meet the consumer's needs
  • Serve as QIDP
  • Provide monthly monitoring of the program
  • Meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines
