About The Position

Case Manager – Forensics: Be the Bridge to Stability FrontLine Service is looking for Case Managers to join our Forensics Team. This isn’t just a desk job—it’s a chance to provide life-changing mental health interventions where they are needed most. You’ll serve as the primary advocate for individuals with severe mental illness as they navigate the Cuyahoga County Jail and Municipal Court systems. You won’t just manage cases; you’ll foster hope and stability for those in the justice system. Your Impact: Advocacy & Support: Navigate community resources to remove barriers and help clients reach their personal goals. Essential Stability: Help clients secure housing, vital ID documents, public benefits, and employment. Clinical Coordination: Partner with clinical teams to ensure clients attend appointments and maintain continuity of care after discharge. Collaborative Justice: Work closely with court and jail personnel, as well as probation and parole officers, to support successful community reintegration. Quality Care: Maintain high-standard clinical records and progress notes.

Requirements

A Bachelor’s degree
Experience with or passionate about supporting vulnerable populations
A valid Ohio driver’s license.
