About The Position

The Case Manager-Emergency Department (ED) serves as a key member of the patient care delivery team in a fast-paced emergency department licensed under Loma Linda University Children's Hospital (LLUCH), a Magnet Recognized academic tertiary care hospital with Level 1 Trauma Designation. This role involves applying clinical knowledge and specialized emergency medicine case management, utilization review, and care coordination methodologies and practices to a culturally diverse and complex patient population. The Case Manager partners with physicians, nurse practitioners, physician assistants, and the multidisciplinary healthcare team. They utilize clinical expertise, discretion, and independent judgment to perform the nursing process (assessment, care planning, intervention, evaluation) in accordance with LLUCH's established Professional Practice Model (Duffy's Quality Caring Theory). Collaboration with the healthcare team is essential for planning care, resource utilization, and ensuring timely discharge from the ED with appropriate referrals. The position requires effective interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism, as well as the ability to work well under stress in a fast-paced environment while performing multiple tasks simultaneously. Participation in an on-call schedule may be required based on business needs, and other duties may be performed as needed.

Requirements

  • Bachelor's Degree in Nursing.
  • Minimum three years of bedside nursing in a hospital setting.
  • In-depth knowledge of case management practice and methodologies as applicable to emergency department and services.
  • Exhibits professionalism in behavior, dress, and conversation at all times.
  • Able to speak, read, and write legibly in English with professional quality.
  • Communicate effectively in person, in writing, and on the telephone.
  • Use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program).
  • Relate and communicate positively, effectively, and professionally with others.
  • Be assertive and consistent in following and/or enforcing policies.
