Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. This role is ideal for professionals who value flexible, community-based work, small caseloads, and the opportunity to make a long-term impact through short-term, intensive support.

Requirements

  • Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field
  • Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
  • Valid driver’s license and reliable transportation required
  • Must be insurable under the agency’s commercial liability policy
  • Must demonstrate knowledge in: Mental illness and treatment, Individual and group counseling, Behavioral health assessments and crisis intervention

  • Experience working with children and families preferred

Responsibilities

  • Deliver intensive home-based case management services in accordance with state mandates and agency policies
  • Collaborate with clinical therapists to support individualized family service plans
  • Document all services provided and maintain accurate client records in the electronic health record system
  • Ensure compliance with confidentiality guidelines and proper handling of release of information
  • Participate in 24/7 on-call rotation as required
