Metrocare programs focus on the issues that matter most in the lives of the children, families, and adults we serve. The School Based Youth Clinical Case Manager II position provides psychosocial/rehabilitative service to children/adolescents and their families with serious emotional disturbance in school, home, or community locations. Services may include, but are not limited to, ongoing assessment of clinical needs and social drivers of health, development of measurable treatment goals and objectives, referral/linkage/advocacy, monitoring, crisis intervention, facilitation of groups and continuity of care. Given the needs of the school and those served, this position is expected to serve children/adolescents/families and school facility face to face or via both video and audio technology for 60-70% of services. The overall goal of this position is to maximize the individual’s potential level of functioning, provide mental health, intervention during school to students and facility and aid in the successful reintegration of individuals into the community. School Based Youth Clinical Case Manager strived to empower youth and teachers to gain ownership in their mental health wellness through education and resources. EMPOWER program will provide awareness, prevention, intervention, and resilience activities to ensure that students and teachers have access to and relate to appropriate and effective behavioral health services through Metrocare in partnership with contracted ISD or academy. MONDAY THROUGH FRIDAY - 8am - 5pm

Requirements

  • Bachelor’s degree in human services with 1 or more years’ experience in MH/MR or related field or Master’s degree with 0 to 2 years’ experience in human services.
  • Basic math skills required.
  • Ability to work with reports and numbers.
  • Ability to calculate moderately complex figures and amounts to accurately report activities and budgets.
  • Ability to apply common sense understanding to carry out simple one or two-step instructions.
  • Ability to deal with standardized situations with only occasional or no variables.
  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Ability to utilize Internet for resources.
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business.
  • Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  • Must have an acceptable driving record.

Nice To Haves

  • Experience with trauma-informed services preferred, Seeking Safety, Illness Management and Recovery, and Motivational Interviewing.

Responsibilities

  • Provide direct intervention and clinical services in a school base setting to clients referred to Metrocare by contracted school.
  • Facilitating-Educational groups for facility, parents, and students
  • Crisis intervention
  • Individual skills sessions conducted during the school day or after school
  • Assisting with the scheduling and collaboration of Telephone and virtual psychiatric evaluations and clinical assessment appts.
  • Performs other duties as assigned.
