About The Position

Under direction, provides direct services for developmentally disabled children and their family members as well as designated others. Administers, participates, and implements program evaluations and activities. Informs and assists consumers, their families, or guardians in service provider selection, service authorizations, and program participation requirements. Facilitates team building for the purposes of service provision as well as community integration.

Requirements

  • Bachelor’s degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling or other mental health related field
  • One (1) year of professional work experience managing services for children with intellectual or developmental disabilities and their families
  • Meet qualifications as specified in the MDHHS Michigan PIHP/CMHSP Qualifications per Medicaid Services as a Qualified Intellectual Disability Professional (QIDP)
  • Individual with specialized training or one year of experience (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience) OR one year of experience in treating or working with a person who has an intellectual disability; and is a psychologist, physician, educator (with a degree in education from an accredited program), social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited licensed professional counselor licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, or a licensed physician’s assistant, or
  • Individual with specialized training or one year of experience (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience) OR one year of experience in treating or working with a person who has intellectual disability and an individual with a human services professional with at least a bachelor’s degree in a human services field.
  • Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines
  • Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel

Nice To Haves

  • Master’s Degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling, or another related field
  • Two (2) years of experience or more working with behaviorally challenged, autistic, and/or intellectually developmentally disabled children
  • Effectively speak, write and understand a language other than English is preferred

Responsibilities

  • Conducts assessments, service planning, and specialized intensive case management for children whose primary diagnosis is intellectually developmentally disabled.
  • Provides family training, non-family training, crisis intervention, advocacy, linking, and monitoring for assigned consumers.
  • Completes Macomb County Community Mental Health (MCCMH) record-keeping requirements, including clinical records, Person-Centered Plan, case notes, budgets, and statistical reports.
  • Provides in-service training and educational activities to MCCMH staff, families, and other professionals.
  • Coordinates with members of other disciplines, agencies, and community providers to ensure timely and accurate service provision.
  • Maintains and updates clinical knowledge by participating in service trainings and other activities in designated areas as determined in consultation with the agency’s annual plan and clinical supervisor or designee.
  • Coordinates with family, aftercare programs, and ancillary support services to secure aftercare.
  • Identifies service needs and coordinates fully integrated plans of service.
  • Monitors customer services through face-to-face consumer contact, facility visits, reviewing records, and consulting with family members, guardians, and service providers.
  • Develops effective relationships with service providers, home staff, and families.
  • Monitors and completes service authorization requests in accordance with policy and Medicaid Manual.
  • Reviews Community Living Support (CLS) notes, develops data sheets, and connects consumers to service provider agencies.
  • Gathers and supplies data and statistics as requested.
  • Promotes consumer community-based integration, and coordinates with family members and ancillary services to secure continued care.
  • Participates in staff meetings, in-service training, and continued supervision with Clinical Supervisor.
  • Operates an automobile to perform assigned job functions.
  • Performs related duties as assigned.
