About The Position

The Case Manager Assistant (CM-A) is responsible for assisting the Case Managers with various healthcare service deliveries and discharge planning with the multidisciplinary teams' input. The Case Manager Assistant will be vital in obtaining necessary resources and distributing documents to responsible stakeholders.

Requirements

  • High school diploma or equivalent required.
  • Two (2) years of case management experience or healthcare experience is preferred.

Responsibilities

  • Collaborate with Admissions Team and Clinical Liaisons to prepare pre-admissions paperwork.
  • Access lab reports, medications, and therapy notes to accompany reports to stakeholders.
  • Fax reports/medical records/clinical records to stakeholders.
  • Upload and file documents accordingly in resident’s chart.
  • Ensure all required documents are in the resident’s record prior to admission.
  • Assist Case Manager in ensuring all appointments such as medical, dental and vision are up to date and scheduled accordingly in a timely manner.
  • Review medical bills for the residents for accuracy, forward to appropriate stakeholders or guardians for payment.
  • Complete discharge satisfaction surveys.
  • Assist with discharge planning for resources and placements, check on placements accepting of payors, bed availability, services provided, cost, etc.
  • Conduct Admissions Assessments as directed by Case Manager.
  • Provide backup for treatment planning schedule/changes & distribution.
  • Assist Case Manager with preparations for Master Treatment Planning Meeting.
  • Complete a six-month follow-up call to all patients/guardians for a status update.
  • Transport residents to and from appointments, as directed.
  • Assist in notifying parent/guardians on updates regarding emergency behavior interventions and serious incident reports as directed by Case Manager.
  • Assist Case Manager in sitting in on meetings with outside team for weekly check-ins, monthly check-ins, or court hearings.
  • Assist in coordinating weekly visitation log for weekend visits with family, friends and case workers as directed by Case Manager.
  • Performs other duties as assigned.
