Case Manager (CPST, QMHS) Case Managers (CPST) facilitate the delivery of services to clients in need in the community, and may assist with independent skills training, advocacy for financial, medical and legal entitlements. and arranges for crisis interventions when needed. They facilitate the rehabilitation process for clients and families; monitoring and will coordinate the delivery of necessary evaluations, assessments to identify the patient needs, development of the rehabilitation plan with patient and significant others, linkage and coordination of appropriate community services. Assertive Community Treatment (ACT) is a form of community-based mental health care for individuals experiencing serious mental illness that interferes with their ability to live in the community, attend appointments with professionals in clinics and hospitals, and manage mental health symptoms. ACT Case Managers are located primarily in Clark County, but some travel to Madison County may also be required. . Behavioral Health Rehabilitation (BHR ) This is a community based program providing services to adults who are struggling with mental health and addiction issues. BHR Case Managers goal is to assist individuals in reaching their highest protentional, recovery and community integration. Adult Outpatient- Adult Outpatient services provide interventive mental health services for adults, and families. Outpatients Case Managers will facilitate and work with clients to achieve maximum independence in the community. Youth Outpatient/Schools - Youth services provide assessment services, counseling services, and service coordination to youth who have emotional, behavioral, family and substance use problems. Youth includes services provided at schools and at our main facility and also community-based services.
