ACT Case Manager provides intensive, community-based treatment, support, and rehabilitation services to individuals in their natural living environments rather than in clinic or hospital settings. Working as part of multidisciplinary team that shares responsibility for client outcomes, the case manager uses assertive engagement to proactively support individuals with serious mental illness. Services are delivered where people live, work, and socialize, with focus on skill building, recovery, housing stability, employment support, family involvement, and wellness self-management to promote community integration and reduce hospitalization.
