About The Position

The Bridge offers supportive housing and behavioral health services to vulnerable New Yorkers facing behavioral health and substance use challenges. As a recognized leader in developing and operating supportive housing and community-based programs, The Bridge has 70 years of experience helping New Yorkers with mental illness, including those affected by homelessness, incarceration, and institutionalization. ACT Case Manager provides intensive, community-based treatment, support, and rehabilitation services to individuals in their natural living environments rather than in clinic or hospital settings. Working as part of multidisciplinary team that shares responsibility for client outcomes, the case manager uses assertive engagement to proactively support individuals with serious mental illness. Services are delivered where people live, work, and socialize, with focus on skill building, recovery, housing stability, employment support, family involvement, and wellness self-management to promote community integration and reduce hospitalization.

Requirements

  • Bachelor’s degree required.
  • Must have or be eligible to obtain New York State identification (non-driver ID or driver’s license).

Nice To Haves

  • Minimum of three years of experience providing services to adults with serious mental illness preferred.
  • Knowledge and experience with electronic health records, specifically AWARDS by Foothold, preferred.
  • Spanish-speaking ability is highly desirable.

Responsibilities

  • Collaborate with a multi-disciplinary team to provide services consistent with the ACT model.
  • Participate in daily team meetings to review and plan client services.
  • Provide the expected number of services to clients each month, with 80% delivered in the community.
  • Complete required documentation in a timely and high-quality manner.
  • Work collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals.
  • Coordinate client care through close involvement with hospitals, substance use treatment facilities, and other providers.
  • Complete immediate needs assessments, comprehensive psychosocial assessments, individualized service plans, and crisis/relapse prevention plans.
  • Use evidence-based treatments in the areas of mental health and substance use to address client needs (e.g., cognitive behavioral therapy, harm reduction, motivational interviewing, trauma-informed care).
  • Provide services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical health care.
  • Provide services to assist clients with needs such as legal advocacy, navigating the criminal justice system, and coordination with legal agencies.
