About The Position

Project FIND’s mission is to provide low- and moderate-income and homeless seniors with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these men and women, Project FIND is a critical resource, providing housing, meals, and programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living. Job Summary: Case Management services help older persons with functional impairments gain access to appropriate services, benefits and entitlements needed to age safely at home and to maintain their quality of life. The Case Management (CM) Program Director (Bilingual preferred), minimally an MSW holder, will have responsibility for supervising 5 case managers and will ensure that all case management clients receive a timely, thorough, strengths-based assessment; and that care plans are developed that reflect culturally sensitive options that make efficient use of community resources. She/He will be responsible for creating a training and development plan for each case manager on his/her team. Included in this plan will be the assurance of completion of all NYC Aging training requirements and continued support of ongoing staff development. The CM Program Director will report to the Director of Community Services and will develop and maintain external partnerships with other organizations to expand resources and strengthen supports. Responsibilities: Administrative Tasks: Master the NYC Aging Case Management Standards and adhere to its protocols/standards. Assign cases to, and close cases for CMs. Approve and sign service plans. Complete supervisory case record review as required by NYC Aging. Ensure that the CMs records maintain up to date information in NYC Aging database. Ensure that required program statistics are submitted in a timely manner each month Ensure timely responses to weather-related emergencies. Ensure timely submission of staff timesheets. Participate in Project FIND staff evaluation processes. This position is not limited to the list above as the supervisor may be required to perform other appropriate tasks as needed. Supervisory Tasks: Provide case managers (CMs) with ongoing supervision (including weekly individual check-ins). Provide ongoing support to CMs when dealing with a difficult client. Ensure case managers obtain the required annual NYC Aging training hours. Train and provide case managers with NYC Aging Case Management policies and Project FIND’s protocols and standards. Track case manager assessments, re-assessments, referrals and follow ups. Accompany CMs on home visits when deemed necessary. Performance Management: Advocate and intervene with lead agency, program partners and other agencies on behalf of the clients and Project FIND. Liaise with the lead contractor’s (Encore Community Services) Director of Case Management and other management staff regularly. Identify, participate and manage NYC Aging’s required training hours as well as agency-wide requirements for self and CMs. Attend all required NYC Aging, Encore and PF staff and/or agency meetings.

Requirements

  • MSW or related Masters degree (eg: social services, public administration, nursing, or public health) LMSW/LCSW (preferred) and 2 years supervisory experience required.
  • Must have 3 years of experience working in social services or related fields.
  • Must have experience working in the field of aging.
  • Must demonstrate cultural competence, be knowledgeable and sensitive to the diverse needs of the communities served, including clients with visual and/or hearing impairments.
  • Must be able to multi-task and prioritize work.
  • Bilingual English/Spanish or English/Chinese speaker preferred and must demonstrate verbal and written competencies.
  • Ability to gather, summarize, and review data for reports, find solutions to challenges related to the clients.
  • Ability to apply principles of logical thinking to a wide range of intellectual and practical problems.
  • Demonstrate excellent interpersonal and communication and strong customer service skills.
  • Must exercise excellent follow-through on assignments.
  • Must have crisis-management skills.
