About The Position

The Case Management Coordinator is a specialized professional position that is responsible for supervising housing focused case management and services for individuals experiencing homelessness at the Embry Rucker Community Shelter (ERCS). The Case Management Coordinator must have strong clinical, interpersonal, and motivational skills and must possess a patient, trauma-informed approach to service delivery using an equity lens. The Case Management Coordinator ensures that housing focused services are equitable and delivered with integrity in accordance with the Agency Mission, Vision, and Values. The Case Management Coordinator is responsible for utilizing a Housing First approach to exit adult-only households from ERCS and is responsible for meeting or exceeding outcomes as well as other contractual obligations. The Case Management Coordinator serves as the primary housing focused case manager for individuals participating in the Medical Respite Program (MRP) located at ERCS. The Case Management Coordinator provides critical linkages and coordination of services as an integral member of the ERCS and Cornerstones team focusing on 5 primary outcome areas: 1) reducing length of homelessness, 2) exits to permanent housing, 3) increase in income, reduction in debt, improvement in credit score, 4) reducing recidivism and returns to homelessness and 5) integrated care management. All duties are performed in a professional and timely manner, in a fast-paced, high-demand environment. The Case Management Coordinator serves as a part of the leadership team of the Housing and Community Services Division. A candidate with an MSW plus two years post-Masters experience and eligibility to supervise social work interns is preferred. A candidate that is bilingual in English/Spanish and has the ability to use one's lived personal experience of housing instability to help assess system barriers is a plus.

Requirements

  • Bachelor’s degree in an applicable human services field or commensurate experience with homeless and/or at-risk populations to include supervisory/management experience.
  • Three years of experience in homeless services, residential services, and/or housing is required.
  • Prior experience supervising and managing homeless services programs (rapid rehousing, emergency shelter, homeless outreach, hypothermia prevention, etc.) is required.
  • Experience working with vulnerable individuals including people with mental health and/or substance abuse disabilities is required.
  • Requires knowledge and belief in “Housing First” and “Rapid Re-Housing” philosophy and strategies.
  • Knowledge of current social service, homeless, and housing issues and methods/approaches to address issues.
  • Knowledge or understanding of tenant’s rights and responsibilities, “strengths based” case management, trauma-informed care, and racial equity.
  • Ability to communicate clearly and concisely, both orally and in writing.
  • Ability to engage and work collaboratively with others.
  • Ability to use HMIS to establish and maintain case records and to facilitate data collection.
  • Ability to schedule and manage workload sufficiently to meet deadlines.
  • Ability to learn, process information, prioritize competing priorities and make sound and reasoned decisions in a fast-paced environment.
  • Ability to complete tasks while navigating frequent interruptions.
  • Ability to work a flexible schedule including nights and weekends.
  • Candidate must be able to lift items weighing 10-20 pounds.
  • Candidate must possess a valid driver’s license, reliable transportation, and good driving record.
  • Candidate must be able to drive 15-passenger van.

Nice To Haves

  • Master's Degree preferred.

Responsibilities

  • Provides direct supervision for ERCS Case Managers, overseeing the provision of housing focused services for individuals served at ERCS including the Medical Respite Program (MRP).
  • Carries a small caseload of adult only households accessing housing focused services at ERCS including the participants accessing the Medical Respite Program (MRP).
  • Provides creative and innovative direction for assessing housing barriers of households who are experiencing homelessness to determine housing and service needs.
  • Manages state and county direct assistance funding streams.
  • Assists in the development of programs and evaluation of all services provided, ensuring the services meet predetermined local, state, federal and agency outcomes.
  • Solves difficult problems with effective solutions.
  • Thinks logically and practically before making decisions.
  • Defines problem areas and develops innovative solutions.
  • Works closely with the Fairfax County Health Department to ensure smooth and effective delivery of services for the MRP program.
  • Maintains effective partnerships with OPEH, the Fairfax County Health Department, Health Works, the Fairfax County Community Services Board (CSB), Projects for Assistance in Transition from Homelessness (PATH), Department of Family Services (DFS), the Fairfax County Department of Adult and Aging, and the Veterans Administration (VA) to ensure shelter participants receive holistic, integrated care management across Cornerstones.
  • Manages the HMIS data, outcomes, and case files for ERCS ensuring that performance measures and outcomes meet contractual obligations.
  • Serves as HMIS Super User, generating reports and monitoring HMIS data for completeness and accuracy.
  • Provides regular case record reviews with case managers and ensures that case records meet contractual standards.
  • Provides clinical supervision for all housing case managers, coaching and developing staff and encouraging staff to bring different points of view to the job.
  • Supervises the practicum experience within the agency for interns ensuring student education goals are met.
  • Participates in multi-disciplinary meetings, case staffing, as well as community-wide trainings and meetings.
  • Participates in agency events, drives as needed.
  • Performs other related duties and responsibilities as required.
