About The Position

The Case Management and Swing Bed Coordinator is responsible for assisting the Director with the management of day-to-day operation of the Case Management Department. The Coordinator creates a common vision to facilitate continuous quality improvement in the delivery of patient care in support of the organization’s mission and values. The Coordinator duties may include managing a daily assignment in addition to a few managerial responsibilities. The Coordinator will assist the Director in providing leadership in clinical case management services to maximize the patient-centered outcomes across the patient-care continuum from pre-admission through post-discharge. Case management services include monitoring patient care to ensure progress-desired outcome, addressing patient and family needs, resolving obstacles to effective care, obtaining authorizations for services from third party payors, and planning for patient discharges.

Requirements

  • An Associate or Bachelor’s degree in nursing is required.
  • If RN, current Ohio license required.
  • Three to five years experience in a healthcare setting.
  • Interpretive skills, frequent interactive and consultative associations are inherent to this position.
  • The ability to retrieve, communicate or otherwise present information in a written, auditory or visual fashion is essential.

Nice To Haves

  • A Certified Case Management designation is desired.
