As a Case Management Assistant, you will facilitate the safe and timely transition from acute care to alternative levels of care such as skilled nursing facility, long-term acute care, inpatient rehabilitation, home infusion therapy, hospice and/or home care program. This position performs tasks identified in the inpatient discharge care plan for management of our patients across the health care continuum. Communicate with patients and all members of the health care team while coordinating services with post-acute providers and medical insurance payors. Arrange transportation and authorizations for the Care Coordination department and deliver regulatory notices as delegated by the Care Coordination Team. -The position requires a proficiency level typically achieved with 2-3 years' experience in healthcare as a Nursing Asst, Medical Asst, Health Unit Coordinator, Placement Specialist, etc. -Must demonstrate effective communication and customer service skills, human relation skills and time management skills with flexibility in responding to multiple demands.
