The Case Coordinator is responsible for providing a range of services to patients throughout their length of stay. These services include clinical psychosocial assessment, treatment, and discharge planning. Direct service is delivered through individual, family, and group treatment modalities. Case coordination involves communication and collaboration with multi-disciplined staff members, the patient, and various collaterals such as family, school, legal contacts, case managers, and managed care organizations across the continuum of care. The primary objective of case coordination is to ensure safe, effective, and solution-focused treatment is provided at the least restrictive level of care possible.
