About The Position

Redwood Coast PACE with Humboldt Senior Resource Center is looking for CarePartners to provide care for our participants in the Fortuna area. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. Shifts available: Monday - Friday 12 PM - 8:30 PM Tuesday - Saturday 8 AM - 4:30 PM JOB SUMMARY: The CarePartner (In-Home) provides direct, compassionate care and support to Redwood Coast PACE participants in their homes, as outlined in each participant’s individualized Plan of Care. Working under the supervision of the Lead CarePartner and in collaboration with the Interdisciplinary Team, this role assists participants with activities of daily living, personal care, light housekeeping, meal preparation, and medication and therapy support as directed. The CarePartner helps promote independence, dignity, and quality of life while ensuring a safe and clean home environment. This position also plays a vital role in monitoring participants’ health status, reporting changes promptly, and fostering positive communication with participants, families, and care team members.

Requirements

  • High school diploma or GED equivalent required.
  • Minimum one (1) year of experience working with a frail or elderly population required, preferably providing personal care in a health facility or in-home setting.
  • Current CPR and First Aid certification within six (6) months of hire.
  • Valid California Driver’s License, and personal auto liability insurance required.
  • Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements, and that can be used for the applicable job functions noted above.
  • Requires clearance of a DOJ and FBI criminal history background check.
  • Requires pre-employment physical, and PPD skin test (TB screening).
  • Requires Motor Vehicle Report verification.
  • Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
  • Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to the community.

  • Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals).
  • COVID-19 and Influenza (flu) vaccinations preferred.

Responsibilities

  • Assists participants with bathing, grooming, toileting, and feeding at home.
  • As applicable and per the program participant’s Plan of Care: Provides escort services to and from participant medical and other appointments.
  • Routinely monitors vital signs such as weight, temperature, blood pressure, and pulse at home.
  • Maintains personal care supplies such as incontinence supplies, nutritional supplements, and personal grooming items.
  • Provides laundry services at home.
  • Provides light shopping for necessary items such as toiletries, breakfast, and snack items.
  • Provides light housekeeping to maintain a clean and sanitary environment in the home.
  • Prepares diet appropriate meals for participants who require assistance at home.
  • Provides nutritional supplements to participants at home as ordered by the Primary Care Provider and directed by the Registered Dietitian.
  • Assists in the monitoring of appropriate diet and food and fluid intake in the home.
  • Monitors that frozen meals are sent home with participants, as required.
  • Implements the Physical Therapy and Occupational Therapy maintenance programs in the home.
  • Assists in the monitoring of participant physical and chemical restraints, adhering to policy and procedures for monitoring and communicating with Primary Care Physician, Registered Nurse (RN), and/or Home Care leadership team as required.
  • Assists participants in the home to get ready for transportation to the Day Center in the morning.
  • Immediately reports any observed changes in a participant’s behavior, health or mobility to the Home Care leadership team, Registered Nurse (RN), and other disciplines as needed.
  • Participates on the Interdisciplinary Team as required and communicates effectively with participants, family members, caregivers, and employees.
  • Completes documentation in the medical record accurately and in a timely manner.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures.
  • Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors, and others.
  • Monitors personal safety and attends to good body mechanics.
  • Ensures infection and exposure control and is prepared in case of emergency.
  • Consistently meets or exceeds Redwood Coast PACE targets for productivity.
  • Consistently meets or exceeds Redwood Coast PACE targets for customer service.
  • Continually seeks better ways for delivering services and communicating with participants.
  • Effectively collaborates with co-workers and contractors to meet Redwood Coast PACE goals and further success of the program.
  • Complies with all policy and procedures of Redwood Coast PACE.
  • Demonstrates proficiency in delivering age-specific care.
  • Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area.
