Redwood Coast PACE with Humboldt Senior Resource Center is looking for two full-time CarePartners to provide care for our participants. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. Shifts we're looking to fill: Tuesday through Saturday: 8:00 am to 4:30 pm Tuesday through Saturday: 12:00 pm to 8:30 pm Compensation is commensurate with experience, credentials, and certifications. Candidates with an active state Certified Nursing Assistant (CNA) license are eligible for a higher rate of pay. JOB SUMMARY: Under the supervision of the Lead CarePartner, provides home care services to Redwood Coast PACE participants in their homes as authorized in the participant’s Plan of Care as determined by the Interdisciplinary Team.
