About The Position

Responsible for coordinating the clerical and administrative processes for entity care transitions management Responsible for managing departmental mailbox, phone lines and/or fax Supports transition planning activities as needed under the direction of care transition managers: Calls vendors, physician offices, clinics, etc.; Faxes (electronic and traditional) documents as needed Schedule follow-up appointments, call Insurance companies for referral approvals for patients Assist/support the Case Managers – will be assigned to 3 to 4 floors of patients – will go into the patient’s rooms for signatures on documents, as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs, walkers, wound vacs) to the patients prior to discharge Makes copies or gathers other data as needed Collects and enters data as needed Submits for prior authorizations for transportation and post-acute disposition via insurance portal. Requirements

  • High School Diploma or Equivalent Required
  • 3 Years relevant experience Required , ( preferably in a healthcare or service environment)
  • Knowledge of medical terminology preferred Problem solving skills Latitude for decision making and setting of priorities
  • Interpersonal skills and Communication skills
  • Ability to work with minimal supervision or without supervision when necessary
  • Proficient computer skills with experience in: MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software
