About The Position
Cheer Home Care is seeking a Care Navigator in San Diego. This role involves creating unique care plans, managing client care, and providing continuous feedback to clients, their representatives, and caregivers. The Care Navigator will collaborate closely with the office team and must possess excellent communication and problem-solving skills.
Requirements
- Upbeat Personality
- Strong Computer Skills
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance (LIC 9163)
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call
Nice To Haves
- Master in Social Work, Nursing or Relevant Field, preferred
- Industry Experience, preferred
