About The Position
Cheer Home Care is hiring a Care Navigator in San Diego. The Care Navigator is responsible for creating unique care plans, managing client care, and providing constant feedback to clients, their representatives, and caregivers. This role requires close collaboration with the office team and demands excellent communication and problem-solving skills.
Requirements
- Upbeat Personality
- Strong Computer Skills
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance (LIC 9163)
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call
Nice To Haves
- Master in Social Work, Nursing or Relevant Field, preferred
- Industry Experience, preferred
