The Care Navigator will play a key role in supporting individuals living with dementia. Once members are enrolled, the Care Navigator will maintain ongoing engagement by completing monthly phone check-ins to ensure continued support and address any questions or needs. This role requires excellent communication skills, attention to detail, and the ability to build trust with members. The Care Navigator will work independently while adhering to compliance standards and program guidelines.
