About The Position

Care Manager RN - Case Management at Providence Petaluma Valley Hospital in Petaluma, CA. This position is Part-Time and will work 8-hour Day shifts. Providence Petaluma Valley Hospital provides outstanding patient care which has garnered our placement on the Maternity Honor Roll for the California Health and Human Services Agency. Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. Requirements

  • California Registered Nurse License
  • 2 years Acute care clinical or comparative experience.

Nice To Haves

  • Bachelor's Degree in Nursing. Required within 3 years of hire date.
  • CCM
  • 3 years experience in community health nursing or case management.
