About The Position

Care Manager RN - Case Management $10,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment. The Care Manager RN provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management RN is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting. Requirements

  • Associate's Degree in Nursing degree/diploma.
  • Texas Registered Nurse License - if practicing within this state.
  • 2 years of Acute care nursing experience in a Medical Surgical or Inpatient setting.

Nice To Haves

  • Bachelor's Degree in Nursing.
  • 1 year of experience in care management or utilization review in any setting or successful completion of TIPS program or Case Management Orientation Program.
