About The Position

The RN Case Manager assesses, plans, implements and monitors a comprehensive plan of care for her/his group of patients. This is done in collaboration with other members of the medical team, i.e. physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary, and nursing. Maintains a focus on the patients progression of care throughout the hospitalization, working with the care team on the appropriate utilization of resources/services and creating a safe, effective and efficient transition/discharge from the hospital.

Requirements

• Licensed RN in Florida.
• 2 years recent RN experience (in a clinical capacity) applicable to the designated population served is required.
• Case Managers hired after January 1, 2018 who do not possess a BSN and/or a MSN will be required to obtain, at a minimum, a BSN within three years of employment as a Case Manager.
• Case Managers hired after January 1, 2018 will be required to obtain specialty certification within two years of employment as a Case Manager.
• Valid RN license in the State of Florida.

Nice To Haves

• BSN preferred.
• Prior Case Management experience is preferred.
• Professional nursing board certification in specialty area preferred.
