About The Position

Support care management teams by helping members receive appropriate healthcare services. This role focuses on member outreach, coordinating care activities, and ensuring continuity of care and member satisfaction.

Requirements

• High School Diploma or GED required
• 1–2 years of experience in a medical, healthcare, or insurance-related environment
• Examples: nursing homes, medical offices, healthcare call centers, insurance support roles

Responsibilities

• Contact members by phone and, where applicable, through home visits to discuss care or service plans
• Explain next steps in care, available resources, and answer member questions or concerns
• Coordinate care activities with care managers, healthcare providers, community partners, and caregivers
• Support member and provider inquiries related to care plans and services
• Assist with basic member assessments or screenings and document care needs
• Maintain accurate and complete member records following state and regulatory requirements
• Communicate updates and concerns to care managers and healthcare professionals
• Identify member needs and refer cases to care managers, disease management teams, or community organizations
• Educate members on health benefits, programs, and available resources
• Follow company policies, regulatory guidelines, and standards of practice
• Perform additional duties as assigned
