About The Position

The Care Management Coordinator plays a pivotal role in supporting the Care Management department by assisting with the coordination, management, and tracking of care management activities. This role ensures the efficient delivery of services and ensures that all projects and tasks are executed on time, in compliance with regulatory standards, and in alignment with department goals. The Care Management Coordinator works closely with case managers, providers, and other stakeholders to facilitate seamless communication and improve overall care outcomes.

Requirements

  • Associate degree, Administrative Assistant certification, or equivalent experience.
  • Minimum of two (2) years of experience in Care Management or Managed Care settings.
  • Advanced proficiency in Microsoft Office Suite.
  • Strong ability to prioritize tasks, work independently, and make informed decisions in a fast-paced environment.
  • Ability to manage multiple tasks and projects efficiently with minimal supervision.
  • Capable of adapting quickly to urgent situations requiring immediate attention or turnaround.
  • Strong organizational skills with an aptitude for multitasking and problem-solving.
  • Excellent communication skills, both verbal and written.
  • Critical thinking and analytical skills, with the ability to make sound decisions.
  • Demonstrated initiative and ability to handle confidential information with discretion.
  • Bilingual proficiency in Mandarin, Cantonese, Vietnamese, or Spanish, Korean is a plus
  • Must be able to travel when needed or required
  • Ability to operate a keyboard, mouse, phone and perform repetitive motion (keyboard); writing (notetaking)
  • Ability to sit for long periods; stand, sit, reach, bend, lift up to fifteen (15) lbs.
  • Ability to express or exchange ideas, to impart information to the public and to convey detailed instructions to staff accurately and quickly.

Responsibilities

  • Assist case managers with member outreach, including scheduling appointments, conducting assessments, referrals, and providing educational materials.
  • Support case managers in ensuring members’ needs are addressed promptly and appropriately.
  • Ensure all documentation, including member assessments, plans, and case notes, is accurate, up to date, and in compliance with all relevant regulations.
  • Organize meeting schedules, training sessions, and project timelines.
  • Prepare materials for meetings and presentations.
  • Monitor the department’s email and fax inboxes, directing messages appropriately and complete member mailings as assigned.
  • Develop and maintain departmental workflows, ensuring they are up to date with best practices.
  • Review processes and identify areas for improvement to increase efficiency.
  • Extract, analyze, and report on data from various sources, ensuring accuracy and consistency.
  • Create tables, graphs, and reports to track progress, identify trends, and measure performance.
  • Request additional data as needed and maintain data integrity across systems.
  • Ensure all necessary regulatory reports and documentation are prepared on time.
  • Coordinate with staff to ensure timely submissions and follow-up actions.
  • Additional duties, as assigned
