About The Position

Devereux Arizona’s Residential Treatment Center (RTC) provides intensive, 24-hour support for youth aged 7 to 17, who are experiencing serious emotional, behavioral, and mental health challenges. In a structured, therapeutic environment, we combine clinical expertise with compassionate, trauma-informed care to help youth heal, grow and build a brighter future. The RTC features dormitories, an accredited K-12 school, food services, 24/7 on-site nursing, and specialized therapy and clinical services. Reporting to the Clinical Program Manager, the Care Coordinator works cooperatively with clinical, operations, medical, and administrative staff on the overall coordination of individual care at the RTC. The Care Coordinator participates in family engagement/outreach, admission/intake processes, discharge and treatment planning, and documentation of all care coordination activities accordingly. The position is responsible for keeping the treatment team informed of assigned cases through team meetings, tracking and communicating due dates of reports, and responding to care management inquiries for clinical staff. Other job duties include: responding to requests for visitations/passes, assisting with admissions processes, clinical documentation, and completing required reports for both internal and external purposes.

Requirements

  • Must be 21 years of age or older
  • High School Diploma or GED required; Bachelor’s degree in Human Services, Psychology, Social Work or relevant field preferred
  • Four (4) years related experience in mental health/social services field with high school diploma/GED OR one (1) year of related experience with Bachelor’s degree
  • Must demonstrate excellent verbal and written communication skills
  • Ability to handle multiple tasks and meet deadlines with minimum supervision; self-motivated
  • General knowledge of clinical best practices for working with adolescent populations and their families/resources
  • General knowledge of mental health, funding and related services within Arizona and other states served.
  • General knowledge of standard office computer applications.
  • Ability to attend two (2) weeks of new hire orientation Mon-Fri 8:30am-5pm (paid)
  • Drug test within 24hrs of offer w/ negative results
  • Pre-employment Physical and Job Demand Assessment
  • Valid Arizona Driver's License & pass DMV 36-month history check
  • Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions). Arizona Revised Statutes Notice (ARS 36-411 Residential care institutions; nursing care institutions; home health agencies; fingerprinting requirements; exemptions; definitions). If a person's employment record contains a six-month or longer time frame during which the person was not employed by any employer, a completed application with a new set of fingerprints shall be submitted to the department of public safety.
