Funded by First 5 San Diego, SBCS’s Healthy Development Services (HDS) program helps south bay families confirm important milestones as their children grow, develop, and prepare to start school. HDS Services include development and behavior check-ups; parent coaching, therapy sessions; and care coordination for families receiving HDS services. Under the supervision of the Healthy Development Services (HDS) Program Director, the Care Coordinator is responsible for providing services to SBCS’ culturally diverse families and/or individuals and the community. Services will be in keeping with the agency goals, objectives, mission and style provided within a team framework. Coordinator is responsible for working with the team to develop team goals and objectives and in meeting those goals and objectives.

Requirements

  • Must have a bachelor’s degree in child development, health, social services, or a closely related field.
  • Minimum of 2 years of work experience in the field of child development, health, social services, or a closely related field. Experience should have preferably been in community-based organization and in the service area of case management.
  • Bilingual (English/Spanish) required.
  • Must be able to adapt to change. Flexibility to programmatic needs is a must.
  • Must be able to effectively communicate and interact with the team.
  • Must be able to multi-task.
  • Strong documentation skills are required with attention to the smallest details in case documentation.
  • Effective verbal and written communication skills. Should be able to adapt communication style to suit different audiences.
  • Must be knowledgeable in the navigation of Zoom platforms, Excel forms, PowerPoint, and creation of Word documents.
  • Applicant should be confident to discuss case discussions in a group format, in person or via Zoom.
  • Ability to work with a diverse population, provide trauma informed services, possess cultural sensitivity, and maintain appropriate boundaries with clients.
  • Candidate is required to have reliable transportation; therefore, the candidate must successfully complete a motor history check. Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy.

Responsibilities

  • Conduct family screenings, intakes, and strength-based assessments with clients.
  • Process referrals for the program’s eligibility criteria, accuracy, and immediate needs of families.
  • Maintain assigned caseload & contact with clients on a regular basis.
  • Develop service plans with clients in accordance with assessment findings, HDS referral, and HDS Providers’ feedback.
  • Provide case management and crisis intervention services to clients.
  • Provide resources and referrals to, or on behalf of clients, and ensure clients are appropriately linked to services within and outside of HDS.
  • Assist families in understanding, navigating, and accessing health and developmental systems and services.
  • Communicate with families after services have been terminated to ensure successful linkages and confirm usefulness of services.
  • Utilize HDS Service Facilitation Funds for clients as needed and assist clients in overcoming barriers to accessing services.
  • Communicate with HDS referring parties, HDS Providers, and other involved professionals on a regular basis.
  • Timely maintenance and completion of all required agency and First 5 Commission case file documentation in accordance with First 5 Commission and Agency standards.
  • Utilize the First 5 Commission’s approved database.
  • Participate in HDS team meetings, training, individual supervision, and clinical group supervision.
  • Participate in HDS Multi-Disciplinary Team meetings when requested.
  • Attend South Region collaborative meetings and resource fairs within the community to build and maintain networks and resource development.
  • Prepare monthly statistics for First 5 Commission reports, as required.
  • Adhere to agency and First 5 Commission client confidentiality, HIPAA, and mandated reporting policies.
  • Attend and participate in all agency required trainings, staff meetings, and activities.
  • Attend community events that may occur on the weekend to promote the Agency and/or HDS program.
  • Other duties as assigned.
