About The Position

As a partner in building brighter futures for youth and their families, your role will be to help individuals and families navigate healthcare, behavioral health, social services, community systems, and support activities that align with The Village Network’s Mission, Vision & Values. A Care Coordinator will serve as the central point of contact for assessing needs, developing care plans, connecting clients to services, monitoring progress, and supporting positive outcomes. The role will focus on connecting clients aging out of the system with community-based services, strengths-based planning, and cross-system collaboration. This is a full-time community-based position that will often require travel for home or community visits.

Requirements

  • Requires a High School Diploma and 3+ years of relevant experience, OR an Associate Degree and 1+ years of relevant experience, OR a Bachelor’s degree in a Human Services field is required.
  • Experience working with children, youth, families, or vulnerable populations in behavioral health, developmental disabilities, child welfare, or related systems.
  • Organizational and planning skills with attention to detail, documentation, and follow-up.
  • Ability to work collaboratively with clients, families, community partners, and multidisciplinary teams.
  • Must be 21 years of age or older.
  • Cultural sensitivity and ability to work effectively with diverse populations.
  • Team collaboration and problem-solving ability.
  • Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
  • Ability to use a computer, proficiency with Microsoft Office products.
  • Basic computer skills and familiarity with electronic medical records.
  • Valid Ohio Driver’s License and maintains a driving record that allows the individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
  • Willingness to travel for various reasons, mainly during the day but occasionally overnight.
  • Ability to visit clients’ homes (may or may not be handicapped accessible).
  • Ability to comply with The Village Network requirements for using restraints, including sufficient strength and completion of extensive training.
  • Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
  • Must be able to lift 20 pounds independently.
  • Must be able to remain alert to problems, needs or emergencies that might arise on any shift, maintaining a safe environment for the clients.
  • Ability to communicate (verbal and written) with all levels or personnel, internal and external to the company.

Responsibilities

  • Conduct comprehensive assessments to identify clients’ needs and strengths.
  • Develop, implement, and monitor individualized care plans with clients and their families.
  • Facilitate care team meetings that include clients, providers, and support systems.
  • Advocate and link clients to community resources, including, but not limited to, healthcare, behavioral health supports, housing, social services, and benefits that will assist clients in meeting their needs, helping to resolve barriers to successful service delivery.
  • Coordinate/Schedule with families and internal staff to assess needs, preferences, and strengths to advocate for treatment.
  • Maintain timely, accurate case documentation and compliance with TVN policies and regulations, along with ensuring adherence to agency, state, licensing, Ohio Rise, and Medicaid program rules and regulations.
  • Provide care services in the following ways:
  • Supporting the client’s efforts toward treatment goals
  • Identify strengths and needs
  • Role model and teach appropriate skills and behaviors
  • Utilize a trauma-focused approach
  • Assist the youth with daily activities
  • Responsible for assisting with crisis de-escalation as needed.
  • Present in a professional and courteous manner in all interactions.
  • Represent the position positively with professionals, families, and community agencies.
  • Coordinate with community partners, care management entities, referral sources, and schools as appropriate to meet client needs.
  • Work directly with stakeholders to build and maintain a positive working relationship for the benefit of your clients.
  • Temporarily assist in transportation needs.
  • Performs other duties as required.
