We are excited to announce an opening for a Case Coordinator who will be instrumental in coordinating aftercare appointments, maintaining communication with referral sources, and keeping legal or external agencies informed of our clients' progress. Requirements

  • High school diploma or equivalent required.

Responsibilities

  • Ensure clients have access to the full continuum of care by assessing their needs and facilitating appropriate services.
  • Coordinate all appointments, aftercare plans, schedules, and meetings.
  • Arrange Halfway House interviews and placements.
  • Obtain necessary information and releases from clients, families, referral sources, and other agencies.
  • Perform other duties as assigned.
