About The Position

Our Older Adult Outreach Program supports adults age 55+ in increasing their independence, safety, and overall well-being. Serving individuals with chronic needs who are not eligible for waiver services, we help older adults remain stable in their homes and communities by connecting them with resources, reducing isolation, and empowering them to live as independently as possible. The Care Coordinator will work directly with older adults to assess needs, develop individualized service plans, and connect individuals to formal and informal community supports. This role is key in helping participants achieve their goals, increase independence, and avoid unnecessary facility-based care.

Requirements

  • Experience working with older adults or individuals with complex needs preferred.
  • Experience maintaining with a base of clients throughout the community required.
  • Strong knowledge of community resources and benefits programs available in Hennepin County required.
  • Ability to build rapport, work independently, and collaborate with a broad network of service providers required.
  • Excellent communication, documentation, and organizational skills required.
  • Valid driver's license, vehicle insurance, and accetable driving record required.

Responsibilities

  • Conduct strengths- and needs-based assessments and develop individualized service plans with each participant.
  • Provide referrals, coordination, and follow-up for a wide range of supports, including transportation, food security, home maintenance, housing resources, companionship programs, benefit navigation, and homemaking services.
  • Facilitate connection to informal supports such as friends, neighbors, faith communities, and other natural resources.
  • Offer one-to-one services or small-group supports to enhance stability, reduce isolation, and promote social engagement.
  • Track progress toward goals, gather required demographic and service data, and document services in compliance with contract requirements.
  • Monitor participants’ ongoing needs and adjust support plans to maintain independence and stability in the home and community.
  • Maintain client records in electronic health system.
