About The Position

Geode Health is a rapidly growing, national provider of outpatient mental health services, with a mission to “Transform mental health by making it more accessible, affordable and effective”. The company is seeking a highly motivated and empathetic individual to join their team as a Mental Health Care Coordinator. The primary responsibility of this fully remote role is to launch a new clinical program that supports high-risk patients, overseeing Care Coordination for their offices in Texas. The successful candidate will work closely with healthcare providers to coordinate and facilitate the implementation of individualized care plans for patients with high symptom severity.

Requirements

  • 5+ years of experience in Care Coordination, Navigation, or Social Work, preferably within a healthcare setting.
  • Demonstrated empathy and strong interpersonal skills, with the ability to connect with patients and understand their unique needs.
  • Excellent organizational and process-oriented skills, including the ability to manage lists and ensure follow-up.
  • Comfortable with technology and electronic health records.
  • Ability to work collaboratively and cross-functionally with various departments.
  • Strong communication skills, both written and verbal.

Nice To Haves

  • Clinical background is a plus but not required.

Responsibilities

  • Reach out to high-risk patients, as flagged by mental health providers, to facilitate the implementation of their personalized care plans, ensuring understanding and commitment.
  • Act as a liaison between patients and healthcare providers to ensure seamless communication and coordination throughout the care process.
  • Manage and organize patient lists, ensuring timely follow-ups and tracking progress.
  • Utilize technology for analyzing patient data and streamlining the care coordination process.
  • Provide ongoing support and guidance to patients as they navigate their care plans.
  • Collaborate with cross-functional teams, including providers, operations, and support staff, to enhance overall patient care.
