Care Coordinators provide support to individuals experiencing serious mental illness (SMI) who are receiving services from the Forensic Assertive Community Treatment Team (FACT). As a FACT team member, the care coordinator works with community resources who provide services, including shelter, housing, detoxification, mainstream benefits, employment, and transportation. Assists individuals in daily living activities and establishing and maintaining linkages that provide community education, support, and independence. Provide case management services for participants, including assisting participants in obtaining housing, coordinating with criminal justice agencies, and assisting clients with activities of daily living such as medication management and independent living skills. Facilitate recovery planning for participants. On-call rotation: Respond to crisis calls for assessment and pertinent intervention. Report program activities and development, crises, and any other pertinent information to the appropriate chain of command. Be available for 24-7 crisis intervention. Provides services within the requirements for the FACT team Participates as an active member of all team meetings. Proactively involve and communicate with collateral supports, including family, significant others, probation officers, judges, caseworkers, and others. Document all incidents immediately and report as required. Complete accurate and thorough client reports in required timeframes, such as court reports. Other duties as assigned.
