As a Full Time Care Coordinator - Educator at Guttenberg Municipal Hospital & Clinics, you will embody and promote our mission, vision, and values while ensuring adherence to all hospital policies, safety protocols, and compliance standards including HIPAA. Your role will encompass driving quality improvement initiatives, engaging in continuous quality improvement (CQI) processes to enhance patient care outcomes, and collaborating with the clinical team to coordinate appropriate levels of care. You'll maintain strong organizational skills, functioning independently under the direction of the PCU manager/CNO, while also demonstrating exceptional communication and customer service with patients, families, and colleagues. With an emphasis on flexibility, you will adapt to evolving healthcare assignments, facilitate training sessions for clinical staff, and participate in departmental meetings and committees. In this role, you will foster a culture of continuous learning by coordinating essential training programs for various certification requirements and contributing to the professional development of nursing staff. To excel as a Full Time Care Coordinator - Educator at Guttenberg Municipal Hospital & Clinics, candidates must possess an Associate Degree in Nursing (ADN) along with a valid and current Iowa Nursing License or Nursing Licensure Compact (NLC). A minimum of two years of clinical nursing experience is essential, with a preference for experience in rural health or various nursing disciplines, including critical care. Candidates should hold certifications in BLS, ACLS, PALS, NRP, and TNCC, with the expectation to complete any additional required certifications within one year of orientation or requirement announcements. A Bachelor of Science in Nursing (BSN) is highly desirable, reflecting a commitment to professional development. Strong organizational skills, the ability to manage multiple databases and electronic medical records (EMRs), and effective communication skills are crucial for coordinating care successfully and fostering an environment of learning and collaboration among staff. Flexibility and adaptability to changing healthcare landscapes are also vital for this role.

Requirements

  • AD in Nursing with 2 years of clinical nursing experience.
  • Valid and current Iowa Nursing License or Nursing Licensure Compact (NLC).
  • BLS
  • ACLS
  • PALS
  • NRP
  • TNCC
  • Certifications to be completed within 1 year of completion of department orientation or if newly required within 1 year of announcement of requirement
