Role Overview The Care Coordinator conducts timely assessments, create and manage care plans, coordinate resources, maintain accurate documentation, and support elderly clients through comprehensive case management and ongoing collaboration.

Requirements

  • Minimum Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 3-5 years relevant experience
  • Driver's license
  • Automobile insurance
  • Minimum IDOA Certification upon hire

  • Preferred Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 5+ years relevant experience

Responsibilities

  • Engage and assess the complex needs of elderly, frail and clients in community settings. Gather information, develop care plans and connect clients to other resources and supports.
  • Conduct assessments for new and existing program participants within established timelines. Provide complex case management services. Develop advanced, well written and effective care plans that appropriately match the needs of the client. Maintain care plans and client confidentiality.
  • Make appropriate referrals, monitors client services, documents services provided within established timelines. Maintain up-to-date client files and databases. Attend required meeting and trainings. Maintain mandatory certification. Required to travel independently throughout assigned communities to complete position duties
  • Work with internal stakeholders as needed.
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.
  • PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.
  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Climb up and down up to 3 flights of stairs at a time
  • Lift, push, and pull up to 25 lbs
