At Children’s Crisis Treatment Center (CCTC), our staff is dedicated to addressing the impact of childhood traumatic events and other challenges. The Care Coordinator will support and follow through with treatment team recommendations in a fast-paced environment and meet the needs of the children ages 5-13 and their families presenting with crisis and behavioral health issues in a trauma-informed manner. Summary of Essential Functions: Completion of all clinical assessments and tools deemed clinically necessary to determine and understand treatment needs and interventions to be employed from the start and throughout the length of stay of services for each child assigned. Implementation of service coordination plan in conjunction with treatment team from Day 1 of start of services for children and families. Complete all required documentation, care coordination notes, of work and outreach completed for each child assigned, up to 10 active and 10 discharged per week. From intake to end of service at the Acute Partial Program, and with the support of the Administrative Specialist, schedule child and family recurring appointments in CCTC’s ECR as follows: 1 Weekly psychiatric medication, MSE, and evaluations, as appropriate, for each child assigned. 2 Weekly Individual sessions per child for their assigned clinician on the team. 1 Weekly family/collateral sessions with clinician per child for their assigned clinician on the team. Identify, link, and coordinate services from beginning through end of care for children and families by communicating effectively with Acute Partial treatment team, CBH, other service providers, and schools. Ensure a warm hand-off to recommended levels of care as part of the aftercare planning by following up with child, family, and service providers within 2-weeks, 30- & 45-days post discharge from the Acute Partial Program. Provide home & telehealth visits for children and families that require this for purposes of engagement, completion of required documentation, and gathering of information otherwise not attained within the program. Complete ModivCare standing orders for assigned children to ensure daily transportation to and from the Acute Partial Program. Meet twice a week with the Acute Partial team (Clinician, BHW’s, Care Coordinators) to review treatment of children reviewing treatments that are working compared with those that are not and follow-up with any treatment recommendations that require care coordination to meet child and family needs while in treatment. Actively participates in the treatment planning meetings reviewing treatment strategies, discharge planning for clients, and other interagency meetings as needed. Demonstrate strong communication and collaboration skills by formation and maintenance of strong alliances with Acute Partial referral sources: CBH, Inpatient Units, CRC, Schools, and other service providers as exhibited by high influx of referrals to the program on a continuous basis. Monthly phone calls and emails to all referral sources. In partnership with the Program Director visit physical sites and have program marketing materials, be prepared to make presentations about the program and referral process. Attends in-service and training meetings as assigned and in accordance with state and CBH regulations. When applicable, reports back information from training in weekly meetings to educate other Acute Partial staff. Immediately employs Sanctuary Model with clients, families and staff and monitors its use in the Acute Partial Program to effectively maintain the Sanctuary Model for all.
