About The Position

The Patient Services Care Coordinator is responsible for organizing and managing all aspects of a patient’s care at TCPC. They will support the surgery center and provider care teams by delivering administrative and patient care services. Their goal is to take initiative to improve the patient’s care and experience. Essential functions of the Patient Services Care Coordinator position include, but are not limited to: Direct contact with patients, both in-person and via telephone. Schedules patient appointments relating to surgery and procedure orders. Acts as direct contact for surgical procedure and injection scheduling. Communicates with the healthcare team about a patient’s treatment plan. Helps patients complete paperwork. Makes phone calls. Coordinates with other departments and outside facilities. Completes pre-procedure screening and education. Assists with prescription prior authorization and completion of forms. Coordinates patient appointments, including diagnostic tests, physical therapy, and pain psychology, in accordance with the patient’s treatment plan. Assists clinical team with patient tracking. Updates patient information in EHR including medications, allergies, primary care physician, pharmacy, etc. Enrolls patients in the patient portal. Coordinates the transmission and acquisition of necessary patient records. Manages all anticoagulant hold requests. Facilitates timely completion of the request. Contacts patient, prescriber, and pharmacy as needed. Cross trains to other departments as needed. Prepares and sends out information packets to patients prior to surgery. Communicates with Medical Device Representatives to coordinate in-person device support. Communicates with Providers and Physicians as needed regarding appointment scheduling and educational materials. Complies with organizational and HIPAA policies and regulations to ensure patient care is kept confidential. Performs miscellaneous job-related duties and tasks as assigned.

Requirements

  • High school diploma/GED plus one year of medical clinic experience.
  • OR Associate’s or Bachelor’s degree (preferred).
  • Minimum of one year of strong customer service with an exceptional ability to listen and communicate with others required.
