About The Position

As a Care Coordinator I, you will facilitate stable living environments for Transitional Age Youth (TAY) clients served by Our Third Place (OTP). You will assess clients' housing needs, collaborate with community partners and service providers, and coordinate resources to secure appropriate and supportive housing options. Additionally, you will work closely with clients to develop individualized housing plans that promote both short-term stability and long-term self-sufficiency.

Requirements

  • Bachelor's degree in psychology or related field.
  • Minimum of one years experience working in a mental health setting with specific experience in housing placement.
  • Have a working knowledge of the housing market, AB34 programs, and placement of individuals that meet the AB34 criteria for placement services.
  • Support the values and mission of Didi Hirsch as related to employment.
  • Know and comply with Agency policies and procedures; HIPAA, Department of Mental Health, licensing policies and documentation guidelines; and other state/federal regulations.
  • Demonstrate current knowledge of all job specific skills including working effectively with homeless or at risk of becoming homeless individuals with a history of mental illness; case management; psychiatric crisis intervention; documentation; multi-cultural and socio-economic issues.
  • Possess demonstrated advocacy skills.
  • Present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Efficiently use the personal computer word processing and spreadsheet software and client database program.
  • Utilize analysis, experience, and judgment to make effective decisions within policies and procedures.
  • Demonstrate commitment to team objectives and Didi Hirsch philosophies
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver's license and a driving record acceptable to the Agency's insurance carrier.

Responsibilities

  • Completes client assessments; provides referrals for a housing match based on clients' psychiatric condition, social support network, substance abuse issues, and readiness for independent living.
  • Documents all services provided and maintains client files, and program statistics as required by Didi Hirsch, contract, and or other related agencies.
  • Provides best match housing placement and assists client with interviewing for housing placement.
  • Interfaces with the Department of Rehabilitation and other related community agencies relative to housing for clients.
  • Gathers/maintains available information and prepares routine reports related to housing placement activity including client placement and successes and challenges to determine trends within the housing market and ensure long term placement for AB34 clients.
  • Consults with staff on service coordination and service plans for clients referred to AB34 program.
  • Monitors housing market activity to identify trends and prospective openings; keeps the AB34 program staff apprised of new housing opportunities.
  • Maintains accurate, complete documentation of services provided to clients, ensures compliance with Didi Hirsch policies and procedures, state licensing, on other regulatory agencies, prepares a variety of reports on housing placement activity for program, division, and Agency.
  • Develops and maintains housing placement skills through seminars, workshops, or other forms of training/education.
  • Attends and participates in routinely scheduled program, division, and Agency staff meetings.
