About the Role The Care Coordination & Forensic Liaison serves as a key bridge between behavioral health care coordination services and the criminal justice system. This position works directly within the local jail to identify, engage, and coordinate care for incarcerated individuals with behavioral health needs who may qualify for Care Coordination, Forensic Case Management, or other community-based behavioral health services.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services or related field required
  • Minimum 2–3 years of experience in behavioral health, case management, care coordination, or forensic services

Nice To Haves

  • Master’s degree preferred
  • Experience working with justice-involved individuals strongly preferred
  • Knowledge of behavioral health crisis systems and community resources

Responsibilities

  • Conduct regular on-site visits within the local jail to identify incarcerated individuals with behavioral health needs.
  • Engage individuals diagnosed with SMI, SUD, or co-occurring disorders who may qualify for Care Coordination or forensic services.
  • Provide education about available behavioral health services and treatment options.
  • Establish rapport and begin engagement prior to release to increase service participation.
  • Conduct needs assessments and begin developing transition plans for individuals preparing for release.
  • Coordinate referrals to behavioral health providers, case management, medication management, housing supports, and other community resources.
  • Ensure continuity of medication and treatment services upon release.
  • Schedule appointments with community providers prior to release when possible.
  • Serve as a liaison between the jail, courts, probation, and behavioral health providers.
  • Collaborate with forensic case managers, court teams, and diversion programs to support treatment engagement
  • Identify individuals who meet criteria for Care Coordination services based on high utilization of crisis or inpatient services.
  • Work with Care Coordination teams to ensure individuals receive appropriate follow-up and support after release.
  • Facilitate warm hand-offs to community-based providers and service teams.
  • Maintain accurate and timely documentation of engagement, assessments, referrals, and coordination activities.
  • Ensure compliance with confidentiality regulations and agency policies
