CentraState Medical Center has an employment opportunity available for a Care Connector supporting the Patient Access Services department. This role involves connecting with patients following their Emergency Department visit to assist in scheduling future appointments tailored to their specific health needs, ensuring continuity of care and improved patient outcomes.

Requirements

  • High school diploma or equivalent required.
  • 1 to 3 years of experience in a health care environment required.
  • Strong professional, organizational, and interpersonal skills to effectively relate to patients and all members of the healthcare team.
  • Ability to manage and prioritize workload required.

Nice To Haves

  • Associate's degree in health related field preferred.
  • Bilingual preferred.

Responsibilities

  • Connects with patients post Emergency Department visit to facilitate future appointments related to their specific health needs.
  • Possesses the knowledge of all hospital services and outside services and able to ‘sell’ and speak to those services.
  • Escalates patient issues when and if needed to the appropriate person suggesting a resolution.
  • Schedules appointments for many different departments.
  • Coordinates follow-up with patients to ensure compliance with future appointments.
  • Collaborates with Emergency Department with health care team to connect with patient at end of visit to optimize patient’s future care.
  • Communicates and documents appropriately each encounter in hospital clinical system.
  • Collaborates, advocates and communicates with departmental leaders on how to improve and update program changes as needed.
  • Motivates and supports patients to become engaged in their follow-up health needs.
  • Tracks and evaluates and statistics related to program.
  • Handles any stat requests if needed and provide follow-up communication.
  • Works with departmental leadership to set goals and monitor goals on monthly basis.
  • Performs other duties as assigned.
