The Care Concierge is on-site at Senior Living Facilities and is a bridge between patients, providers, and Curana’s virtual support team.

Requirements

  • Registered or Certified Medical Assistant.
  • 1+ years of experience working in a medical office, community engagement, or other related fields.
  • Ability to interpret medical records, lab results, and appointment notes.
  • Demonstrates experience in collecting clinical information and surfacing changes to the appropriate Curana care team member.
  • Willingness to travel to local Senior Living Communities.
  • Proficient computer skills and exhibits knowledge in using various Microsoft Office applications.
  • Excellent organizational and time management skills with the ability to prioritize tasks.

  • Experience working with chronic care management (CCM) or other care management programs.

Responsibilities

  • Conduct monthly in-person rounds for patients enrolled in Chronic Care Management (CCM) or the Guiding an Improved Dementia Experience (GUIDE) programs
  • Works on-site within the facility to identify and document resident status changes such as: deceased residents, hospitalizations, rehab placements, and discharge home
  • Identify and enroll new residents into CCM services
  • Introduce CCM programs to residents and caregivers….obtain consent using approved verbal consent template
  • Conduct comprehensive chart review for each resident
  • Complete the CCM Pre-Visit Template based on clinical findings
  • Address patient and durable power of attorney (DPOA) inquiries via telephone.
  • Respond to patient or caregiver messages received via the Curana Patient Portal.
  • Assist patients with scheduling follow-up appointments with Curana Providers or specialists.
  • Manage electronic health records (EHR) and ensure accurate and up-to-date patient records.
  • Coordinates regulatory SNF visits by communicating with facility staff and Curana providers
  • Coordinate documents needed for review or signature by a provider.
  • Facilitate provider orders and escalate findings.
  • Support Provider scheduling.
  • Assists with prior authorizations.
  • Obtain patient records and diagnostic test results.
  • Organizing telehealth consultations.
  • Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within our health network.
