About The Position

Ebenezer is seeking a Campus Assistant Director of Nursing (RN) for our Skilled Nursing Community in Wyoming, Minnesota! This role is responsible for continuity of nursing care by delegating, directing, and monitoring nursing care delivered.Demonstrates management skills through effective problem solving and decision- making, assuring compliance with State and Federal regulations as well as Ebenezer Quality standards. This is a full-time, Monday-Friday 8AM-4:30PM with an on-call rotation. Meadows on Fairview TCU is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch. We offer a home like atmosphere where you feel like you belong the minute you arrive! Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.

Requirements

  • Vocational/Technical Training or Associate Degree - Registered Nurse
  • Current MN RN license
  • Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross; accepted certifications as follows - AHA: BLS for Healthcare Providers (CRP & AED) Program or BLS Provider - BLS Instructor. American Red Cross: CRP/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED
  • 1-3 years of experince in Long Term Care
  • Required to work on-call as needed
  • This position assumes 24-hour responsibility as directed
  • Knowledge of and sensitvity to the needs of the geriatric population

Nice To Haves

  • 3-5 years of experience in Long Term Care
  • Basic computer skills

Responsibilities

  • Leads, supervises, directs and/or delegates all nursing functions on assigned unit or floor to assure that care is provided per individualized plan of care.
  • Assures staff compliance with State and Federal regulations, Nursing Standards of Practice and Ebenezer Quality Assurance Standards: Assures that reporting process is accurate, up to date, and provides cintinuity of care from shift to shift
  • Observes and evaluates cares given by nursing staff through daily rounds, random audits (i.e. assures that nursing staff are completing assignments/documentation, for completeness and accuracy)
  • Monitors mealtimes to assure that : a)appropriate staff coverage and assistance is provided b) proper diets are served in a timely manner
  • Assures that medications and treatments are administered per physician orders and facility policies
  • Evaluates staff performance and address concern promptly and appropriately
  • Coordinatoes and leads team meetings
  • Lead in emergency situations
  • Assists with investigations and follows-up on all incident reports, medications errors, and Vulnerable Adult reports
  • Actively participates in audits to assure quaility care and practices
  • Participates in QAPI and quality improvement projects
  • Offers feedback to Administrator and DON regarding areas of concern
  • Serves as a positive role model to staff
  • Paticipates in MDS assessments/Care Planning as required by State and Federal law: Assists with maintaining and assures accuracy of care plan Acticely incolved at the IDT meetings
  • Participates in the Admissions and Discharge process
  • Assures that resident family and physician are notified of any changes in plan of care
  • Assists with completion of pre-adminission and admission assessments
  • Assists with completion of 90-day assessments
  • Ensures completion of post-fall assessments
  • Assists with managing infection control in facility: Track resident infections Oversee and Track employee illnessess Oversse and ensure resident and employee infection control logs are up to date Work with Coporate support nurse for most recent policies and pocedures around infection control and antibiotic stewardship Track and update resident immunication logs Antibiotic stewardship and sustainability
  • Serves as facility supervisor and assumes administrative responsibilities as assigned: Serves as emergency contact/lead for the facility for all departments Monitors delivery of care throughout the facility Conducts tours for families and potential residents as requested Displays ingenuity in anticipating and meeting the challenges of unexpected situations
  • Particpates in discipline/corrective action process
  • Effectively represent Nursing Administration interpreting facility policies and procedures for staff, residents, and visitors: Monitors Nursing Department Assists Director of Nursing in planning/implementing of department meetings/systems and processes Serves as Director of Nursing designee when Director is unavailable
  • Collaborates with Social Services to determine if potential residents' needs can be met upon admission Assists with hiring process as needed Assists with staffing matters and ensuring positions on schedule are filled as needed
  • Other duties as assigned and directed: Participates in management meetings as assigned Participates in Nurse on-call weekend rotation per facility protocol Meets with Administrator and Director of Nursing as directed Performs other duties as assigned and directed
